
The trustee for Kincumber General Practice unit trust ABN 92 571 704 060 is trading as The Surgery Kincumber at 80 Avoca Drive, Kincumber NSW 2261. It is an accredited private General Practice renting out practice space for visiting independent General Practitioners. The practice also employs General Practice trainee Registrars, practice nurses, and administrative staff.
For the purpose of providing clinical care we may need to obtain some identifying information. The information requested would include some form of official identity, your medicare and or pension eligibility information. Your medicare and pension entitlement would need to be periodically updated as the information changes. We may need to send your identity information to your previous health provider for the purpose of them to authenticate the request for transferring your information, however, we do not directly store your identity documents. Your medical records, which would include the clinical notes from the doctor at your visit, correspondences as well as investigations and or other relevant documents would be stored within the electronic patient management system.
Where it is lawful and practicable to do so, you may interact with us anonymously or using a pseudonym — for example, for a general enquiry that does not require us to provide you with a health service. However, because we are a medical practice, most of our services require us to know who you are in order to provide safe care, meet our legal obligations, and to facilitate Medicare rebates.
All information are stored securely in the Electronic Medical record as per standards within Australia. The storage and handling of the information is subject to Australian Consumer and Privacy laws. We are duty bound to protect information safely and diligently to the best of our abilities. We are mandated to immediately notify of any breaches if they were to happen. Some information is also stored under partner agencies that assist us such as online booking services. Their privacy policy would have been provided to you at the time of your contact with them.
Identifying information may be routinely released while claiming rebates from medicare, or processing payments through payment gateways. These routinely happen following consultation. Your identifying as well as health information will be released when sending referrals to other members of your treating health care team, such as specialists. There may be instances where we release information to third parties such as employers/insurance companies/other treatment providers in the setting of a workers compensation injury. Your information is generally not released without your explicit consent. We may release your information without your consent when summoned by courts or when appropriately demanded by law enforcement as per legal requirement. We will attempt to notify you if possible in those circumstances if permitted. We may use your de-identified information for performance and/or quality improvement processes. You can request to opt-out if you desire by notifying us.
The practice records are held in our practice management software and is located within our premises/locally hosted servers. The retained information as well as backs are all always maintained within Australia and there would be no overseas exposure. The data storage is subject to Australian information storage laws.
The Surgery Kincumber will only contact you for specific reasons. These may include SMS/Phone calls to remind you of upcoming appointments, SMS messages regarding any reminders for medical needs. We also send SMS messages for any health promotion campaigns. (You can opt out of those). If opted in, we may send information about your results. Please contact reception for further information.
Your file held in the electronic medical records is the property of The Surgery Kincumber. You are entitled to access a copy of your records at any time. A health summary (usually 1-2 pages) is provided free of charge. A copy of your entire medical record can be obtained in printed or electronic format for the relevant fee.
If you believe any of the personal or health information we hold about you is incorrect, incomplete or out of date, you can ask us to amend/update the same. If for some reason we are not able to amend/or update we will inform you. As we are legally required to retain patient health records for a minimum period, we are not able to permanently delete or erase your records before that period has passed.
The electronic medical records is compartmentalised for views. Usually the reception/administration staff have no access to your clinical information and may not be able to advise you on any clinical information. For further privacy, eg, restricting your records with access only to your own treating doctor or password protecting your records may be possible. As much as these may provide additional protection, this can sometime result in addressing some urgent clinical issues such as following up on results or correspondences when your regular doctor is away.
The current recommendation is government's My Health Record system has become "opt out" instead of "opt in". Hence we may time to time send an electronic health summary to your "My Health Record" folder. This would be accessible by you via My.Gov
We do not collect any information directly. There are parties involved between you as customer accessing the web page and the content built and maintained by us. These include, from our end, the registrar that have registered this website address, the website hosting service, website performance and security providers, search engine providers and other data analysis agents. From your end, it can be from your device manufacturer and their affiliated organisations, browser plugins, your Wifi provider, the ISP and/or the VPN provider (if in use). The information obtained may include but not limited to: IP addresses, device, browser, geographic locations, ISP related information, time of access, duration of access and personal information may be collected by them in identified or de-identified form for quality control as well as monetising purposes. The only agency that we have directly engaged with is Google (Alphabet inc). We may be provided with de-identified information for performance and quality control of the website from Google (Alphabet inc) our website hosting company as well as security providers. We do not get access to any personal information from any agency.

Our social media accounts are configured for business usage. We get information about (number of) users and user engagement. However, your comments/ messages are visible to the public. The social media spaces are not controlled by us and we do not accept any responsibilities of consequences from your social media engagement.
